Boy Scout Required Forms - Required for all outings. The first two forms in the table above should be very familiar to all Scouts and Scout Parents. The troop Permission Slip and the Medical Form are required to be filed with the adult trip leader before any troop outing other than troop and patrol meetings. The adult leader must carry these on the outing. I usually fill out these forms once for the year and make copies; For any trip, I grab a copy of each, fill out the trip specific stuff on the permission form, and turn it into the adult trip leader along with any required funds. Remember, if there is a medical emergency, the adult trip leader must have ready access to permission slips to authorize medical treatment for your son.
Adult Required Forms - The next two forms in the table below are for Parents and Adult Scout Leaders participating in events and outings. Again, see the Guide to Safe Scouting for detailed policy regarding Medical Forms and Tour Permits.
